Initial Visit  #2001 . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .
Come prepared to discuss the problems you are dealing with, how you've attempted to cope with them up to now, and how you will know when things are better.  This first visit usually takes longer and an hour is set aside to cover most pressing issues.

$130 per 60 minute session.

The remaining visits are usually 45-50 minutes for individual, marital, or family visits.     

             

$110 per 50 minute session
 

Adult ADHD Assessment & Counseling  #2003 . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .
This usually involves testing and interpretation of results. Process yields a written report in most cases. Average sessions needed is three (3).

$130 per session ($50 for summary report)

Telephone Counseling & Consultations  #2004 . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . . .    

Two things to keep in mind:  1) You have to be an existing or former client; and 2) an adult (over 21 years old).  You may know that I have always offered telephone services in urgent or unusual situations or for people in crisis.  However, I have recently established counseling and consultation services over the phone as a matter of routine.  {Please read this before your first session}

Myers-Briggs Type Indicator (MBTI) This test offers a foundation for understanding individual differences and applying that understanding to the ways people think, communicate, and interact. You can take both the initial and advanced (Step II) version of this personality assessment online.  Once completed, get your results and interpretation either in office or via telephone.  $100 - Includes assessment & 45 minute session for interpretation of results.

Brown ADD Scales (Adult version) Ever wonder if you have Attention Deficit Hyperactivity Disorder?  Quickly screen for reliable indications by examining your perception of cognitive symptoms. Take this initial screening assessment online and have results interpreted for you by Dr. Haddock.  Use the results to determine if further assessment is indicated.  $ 75 - Includes assessment & 45 minute session for interpretation of results.

Substance Abuse Subtle Screening Inventory-3 (SASSI-3)  A brief and easy to use screening measure that helps to identify individuals who have a high probability of having a substance use disorder. Get an initial screening assessment to see if it has progressed to the point of becoming an addiction.  All communications are confidential.  $ 75 - Includes assessment & 45 minute session for interpretation of results.

Extended sessions are available at the office.  Charges are $25 for 15 minute increments after the first 50 minutes.

Note:  Dr. Haddock is associated with several employee assistance programs (EAPs) and a few managed care companies.  To avoid out-of-pocket expenses, check first with your employer to see if he is an affiliate EAP provider or on your panel of preferred insurance providers.
